Registrations are in person only. 

Registration can be completed in person at the Religious Education office. We look forward to starting our new adventure in our upcoming faith formation year with you and your family.

For those students receiving sacraments this coming year, the following sponsor/godparent documents are required:

If single

Canon Law: 875,874,892,893, you must be a person suitabale for this role, selected by the candidate's family, and have the intentions of filling this role.

Requirements for the catechetical year can vary if you are seeking the Sacrament of Confirmation for the purposes of marriage. Please see Deacon Juan Antonio & Lorena Rodriguez first for more information. (909)991-8690 or  (909)874-4612

***If student is 18 years or older, they must register in person ***

 Important

  • If in previous year, student has taken Faith Formation classes elsewhere, a transfer letter is needed. 
  • A student may only miss a maximum of 3 classes per year, otherwise the year will have to be made up the following year.
  • If student is a minor, parent must be present with student during onine class session.
  • Parents and Adult students must attend parent orientations and meeting conferences.

Please start the registration process by completing a registration form for each person starting faith formation for the 2023-2024 catechetical year ( Found in the Office of Religious Education). A birth certificate for each student submitting a registration form is needed, along with any sacramental certificates from sacraments they have received. When we have collected the registration form, birth certificate, sacramental certificate(s), and deposit.
